Use Your Biometric Screening Form during your Annual Wellness Exam

You and your covered spouse can meet your Biometric Screening qualification by completing the Biometric Screening Form during your Annual Wellness Exam. Follow the instructions and submit the form as soon as possible prior to the April 1, 2027, deadline.

Make sure all screening values are entered, and the form is complete. Both you and your medical provider must sign the form. All fields on the Biometric Screening Form must be complete, including email and signature. If any information is missing, Propel Support will notify you to re-submit a completed form. Avoid delays by working with your medical provider to ensure the form is complete and keep a copy for your records.

Do I have to wait one year since my last annual wellness exam?
No, you are eligible for one annual wellness exam at any time during the plan year July 1, 2025 – June 30, 2026.
From the SPD:

  • One routine physical examination per plan year.
  • One routine mammogram per plan year.
  • One routine gynecological examination per plan year.
  • One routine pap smear per plan year.
  • One diagnostic screening for prostate cancer, including digital rectal examination and prostate-specific antigen (PSA) test, for:
    • Asymptomatic men aged 50 and older
    • Men 45 and older who are at a high risk for prostate cancer.
