Membership eligibility requirements of Nexus SD

Eligibility criteria:

• 501(c)3 organizations or equivalent, government entities, community/civic/service groups such as:
  • Shelter, food, and clothing programs.
  • Emergency services.
  • Health and human services, including primary healthcare.
  • Advocacy, support groups, disability services, youth programs.
  • Religious organizations.
• For-profit businesses providing critical health or human services, provided they offer free or reduced cost services or accept Medicare/Medicaid.
• Unique cases:
  • Member only service organization are evaluated for eligibility.
  • New organizations operating for less than six months are appraised case-by-case.

Membership requirements:

• Signed Nexus SD Partner Agreement.
• Signed Nexus SD Associate Agreement (for non-HIPAA covered entities) or Findhelp Agreement (for HIPAA-covered entities).
• Must engage in sending/receiving referrals with eligible organizations.
• Must identify a site administrator.
• No membership fee currently.
