Membership eligibility requirements of Nexus SD
Eligibility criteria:
- 501(c)3 organizations or equivalent, government entities, community/civic/service groups such as:
- Shelter, food, and clothing programs.
- Emergency services.
- Health and human services, including primary healthcare.
- Advocacy, support groups, disability services, youth programs.
- Religious organizations.
- For-profit businesses providing critical health or human services, provided they offer free or reduced cost services or accept Medicare/Medicaid.
- Unique cases:
- Member only service organization are evaluated for eligibility.
- New organizations operating for less than six months are appraised case-by-case.
Membership requirements:
- Signed Nexus SD Partner Agreement.
- Signed Nexus SD Associate Agreement (for non-HIPAA covered entities) or Findhelp Agreement (for HIPAA-covered entities).
- Must engage in sending/receiving referrals with eligible organizations.
- Must identify a site administrator.
- No membership fee currently.
