South Dakota Department of Health

Nexus SD is a public-private partnership between the South Dakota Department of Health (SD DOH) and participating partner organizations. SD DOH performs the business, operation, fiduciary, and oversight functions of the statewide information exchange network.

Nexus SD is led by the Community Engagement Director employed by the SD DOH. The Director functions as the main point of strategic and operational integration between the SD DOH, Bureau of Information and Telecommunications (BIT), Nexus SD member organizations, contractors, and other governance structure members. The Director has responsibility for day-to-day operation. The Director also ensures strategic, operational and financial alignment with Federal, State, and regional activities. In addition, the Director leads the Nexus SD Steering Committee.

Helpline Center

Helpline Center is a statewide service provider of critical programs for all South Dakotans that include the 988 Mental Health Crisis Line, 211 Helpline, suicide prevention and aftercare, substance use care coordination, social care technology infrastructure, and other support programs. The South Dakota Department of Health and Helpline Center have partnered on many projects over the years such as the Covid-19 information line, WIC registration, All Women Count!, suicide prevention programming, and most recently, Nexus SD.

Due to its deep community connections and its experience with closed loop referral systems, the Helpline Center has led community engagement efforts alongside the Department of Health for the Nexus SD project.

Findhelp

Founded over a decade ago to provide an easier way to find and connect with social services electronically, Findhelp has become the largest network of free and reduced-cost programs, spanning every zip code across the United States. From federal to local resources, Findhelp ensures that support is accessible in bustling cities and the smallest towns.

As the technology vendor for Nexus SD since 2023, Findhelp has played a pivotal role in providing a seamless and user-friendly experience. They go beyond technology, offering expert support for community-facing navigation and partner organizations.

South Dakota Health Link

South Dakota Health Link (SDHL) is a multi-state Health Information Exchange that delivers critical health information at the point of care. SDHL allows providers to see clinical information about their patient from across the entire SDHL network, allowing clinical information to follow the patient. Having access to this information allows the care provider to have the most up-to-date picture of their patient’s care including major health conditions, allergies, medications, diagnostic results, and filled medication information. SDHL also provides access in real time to valuable alerts for Emergency Department or Inpatient admissions allowing providers to engage with their patients in a more timely and effective manner. 

SDHL and Nexus SD work together closely to ensure their users have a full picture of the individuals they serve from both a healthcare and community service perspective.
